TL;DR
Smoking is generally not allowed in hotel rooms, but permitted outdoors and near entrances. Restaurants allow smoking at outdoor tables only.
At a popular multi-level café with a queuing system, lights turn on at dusk and crowds gather. You take a number from a terminal, then find an unmarked person in the crowd who stamps your ticket — without the stamp you won't be let into the main lower level. Most visitors come just for photos and leave quickly.
Smoking is allowed everywhere outdoors. Hotel lobbies typically have a designated smoking area with ashtrays on tables. Inside restaurants smoking is prohibited, but outdoor terrace seating allows it.
Hotel rooms in Egypt are generally non-smoking. Some hotels explicitly allow smoking in rooms and mark this in their listings. In practice, smoking near the room entrance or on hotel grounds is often tolerated — staff rarely intervene and no-smoking signs are not always posted.
