TL;DR
For an island-wide trip, a rental car is usually more practical and can cost around US$40 per day; add at least an hour to Google Maps travel estimates. Driving a tuk-tuk requires confidence in busy traffic and obtaining local authorization based on an International Driving Permit.
Around Hikkaduwa, Galle and Unawatuna, roads are reported to be in decent condition but narrow and generally undivided. Buses may encroach into the opposite lane and traffic can be busy, so travelers without motorcycle-riding experience are advised not to self-drive a tuk-tuk.
Rather than driving a tuk-tuk yourself, use tuk-tuk taxis or rent a car. One reported car-rental benchmark is about US$40 per day.
To self-drive a tuk-tuk, travelers need an International Driving Permit and must then arrange local authorization in Sri Lanka based on it.
For routes through central Sri Lanka, a car is recommended because distances are substantial and two-wheel travel can become tiring. Add roughly one hour to the journey time shown by Google Maps.
