TL;DR
Visa on Arrival costs $30 and is issued at a counter right after you exit the plane at Denpasar airport. Transit passengers don't need a visa at all.
Note that the visa and the electronic arrival card (QR code) are two separate things. The visa is done on arrival at the airport, but the arrival card is better filled out online in advance, since it may be checked at the departure airport too.
No need to stress about pre-arranging an e-visa online — it's easier and just as fast to do it on arrival. Even with a visa already in hand, baggage claim still takes 20-30 minutes, so you won't save time by pre-applying.
You can handle all the paperwork right at the airport after landing — there's a dedicated counter near the exit that's hard to miss. Visa on Arrival (VOA) costs $30.
If you're on a transit flight through Bali, no visa is required. If your connecting flight is delayed, the airline handles the transfer arrangements, so booking a transit itinerary is safe.
