For standard entry to Vietnam on an e-visa or visa exemption, there is generally no universal standalone pre-arrival card: travelers mainly need a valid passport and, where required, an approved e-visa. If an airline or border point asks for a form, use your actual entry status and first accommodation address.
A pre-arrival card is advance travel information, not a customs declaration. It normally asks for passport details, flight information, travel dates and your first accommodation address in Vietnam.
When entering under visa exemption, select an option such as “visa exemption” or “visa-free” if available. There is no visa number or issue date to enter for this type of entry.
If you are entering on an e-visa, use the number and issue date shown on the approved e-visa document. An arrival-card field does not issue a visa; the e-visa must be arranged separately before travel.
Use a genuine address in the form—normally your first hotel, apartment or other place of stay after arriving in Nha Trang. It may differ from the address originally listed in your visa application if your plans have changed.
Do not enter a random address just to complete the form. Keep the booking for your first accommodation and use that address; your hotel or host is expected to register your temporary stay after check-in.
